چکیده

This thesis is an investigation of the possibilities of realtime surgical simulation, specifically within the domain of congenital heart diseases. The problem domain of surgery on the heart of an infant is presented and the primary information necessary for a pre-operative surgical simulation tool is identified. Through cooperation with surgeons, domain specific abstractions and simplifications with respect to soft tissue simulation is identified and used in the investigation of models for tissue deformation. In general it is not possible to calculate the deformation of tissue precisely in realtime. We therefore need to use simplified elastic models that approximate the real behavior of tissue. The most popular models for realtime interaction and response; the Finite Element model and the Spring Mass model are presented and discussed with respect to to surgery on the heart of an infant. A number of variations and extensions of the classic Spring Mass model are investigated to identify the characteristics that can use the domain specific knowledge of tissue deformation and interaction in cardiac surgery. An elastic model and a geometrical model build specifically for cardiac surgery simulation is developed and presented in this thesis. The thesis is done in cooperation with surgeons from Århus University hospital. A surgical simulator supporting the techniques discussed in the thesis has been evaluated with surgeons for pre-operative use. Furthermore the elasticmodels under investigation have been compared and validated formally with respect to their behavior of deformation.

Realtime Integral Videography Using Intra-operative 3-d Ultrasound for Minimally Invasive Heart Surgery

This paper presents a realtime threedimensional surgical navigation system using Integral Videography (IV) with intra-operative ultrasound data acquisition for minimally invasive heart surgery. We used a trans-vaginal ultrasound transducer with volume data acquisition rate of 5 volumes/second. Experiments using a human hand as phantom showed that the online IV image frame rate is around 2 frame...

Appointment process of open-heart surgery in Shahid Rajaei Heart Hospital using a discrete event simulation approach

Background: The waiting list is a list of selected patients in the surgical queue. If demand exceeds capacity, the waiting list grows rapidly, which may lead to unacceptable waiting for patients, especially those in need of acute medical care. Patients waiting for heart surgery are placed on the waiting list for surgery, and sometimes the waiting time is longer than patients expect. Reducing th...

Realtime Cine MRI in Mice with a Single-Shot EPI Sequence and The Karhunen–Loeve Transform

Background and Aims: Mouse models of cardiac disease play a major role in cardiovascular research but are frequently imaged with echocardiography rather than with MRI. While several groups have shown that MRI can image the mouse heart with an unparalleled level of breadth and detail, many in the cardiac research community remain concerned by the complexity and time-consuming nature of cardiac M...

Bed Capacity Planning Using Stochastic Simulation Approach in Cardiac-surgery Department of Teaching Hospitals, Tehran, Iran

BACKGROUND To determine the hospital required beds using stochastic simulation approach in cardiac surgery departments. METHODS This study was performed from Mar 2011 to Jul 2012 in three phases: First, collection data from 649 patients in cardiac surgery departments of two large teaching hospitals (in Tehran, Iran). Second, statistical analysis and formulate a multivariate linier regression ...

Prevalence and Causes of Mediastinal Reexploration for Excessive Bleeding after Cardiac Surgery Procedures

Introduction: Postoperative bleeding in cardiac surgery is not an uncommon complication and can be evaluated with surgical and nonsurgical causes. Although any type of coagulopathy should be treated before, during, and after the surgical procedure, cardiac surgeons should have perfect surgical techniques for step by step hemostasis to minimize blood loss. Material...
